Open Dental is unusually good at this: the reports carry address fields, and there is a query system underneath for anything the standard reports will not do. Menu wording moves between versions — the paths below were checked against the current manual.

Start in Standard Reports
The report lists live under Standard Reports, in the Lists section. Active Patients is the one built on visit dates: the manual describes it as listing active patients who had a completed procedure within a date range.

Understand which way round the filter works
This is the part that catches people. Active Patients gives you everyone who did come in during the range — the opposite of what you want. Run it for the period before your cut-off to see who was a patient, or use a query for people not seen since a date, which is the direct route.

Use a User Query for "not seen since"
The manual's query examples include one that lists patients not seen since a specified date, and another that lists Inactive or Archived patients with the date last seen. The User Query window runs these and is the tool for anything the standard reports cannot filter.

Keep the status filter honest
Patient status in Open Dental can be Patient, NonPatient, Inactive, Archived, Deceased, Prospective or Deleted. Deceased and Archived should not receive a reactivation card; decide deliberately whether Inactive should.

Confirm the address columns are in the output
The Patients report carries Address, Address 2, City, State and Zip. Whichever report or query you settle on, make sure those columns are actually in it before you export — a list of names you cannot mail is the usual outcome of a recall report built for the phone.

Export as CSV
Query results export as a comma-delimited CSV file, and standard reports can be printed or exported after running. CSV is what you want here.
Once you have the file
Check the export actually carries street, city, state and ZIP — recall reports are built for phoning people and often ship without them. Then clean it before you upload: collapse households, drop anyone flagged do-not-contact, and repair any ZIP your spreadsheet stripped a leading zero from.
